This role is responsible for conducting in-home client assessments, supervisory visits, and care plan oversight for clients receiving home care services, including those utilizing Veterans Affairs (VA) benefits and other insurance programs.The RN will ensure high-quality, compliant care delivery through regular assessments, documentation, plan-of-care updates, and coordination with internal staff and external partners.Key ResponsibilitiesConduct in-person comprehensive in-home assessments for clients at minimum every 60 days, or as required by payer guidelines.Perform supervisory visits to monitor quality of care and caregiver performance.Complete, submit, and maintain all required clinical documentation accurately and within established deadlines.Develop, implement, review, and update Plans of Care (POC) based on client needs and changes in condition.Ensure compliance with VA Community Care Network (CCN), insurance, and agency requirements.Collaborate with care managers, schedulers, caregivers, families, and healthcare providers.Monitor client health status and report changes to appropriate parties.Participate in care coordination with physicians, VA case managers, and insurance representatives.Provide education and guidance to caregivers regarding client care needs and best practices.Support quality assurance initiatives and agency audits.Maintain confidentiality and comply with HIPAA regulations.Participate in orientation, training, and ongoing competency requirements as needed.Respond to urgent clinical concerns within scope of role when assigned.VA & Insurance Compliance ResponsibilitiesEnsure assessments and documentation meet VA and payer standards.Submit required reports and forms in a timely manner.Maintain familiarity with VA Community Care policies and authorization processes.Assist with reauthorizations, recertifications, and service renewals when applicable.Participate in audits, reviews, or compliance requests related to VA-funded services.Communicate effectively with VA case managers and third-party administrators.QualificationsActive, unrestricted Registered Nurse (RN) license in the state of Colorado.Minimum 2 years of nursing experience, preferably in home health, home care, or community-based care.Experience working with VA benefits, insurance providers, or managed care organizations preferred.Strong clinical assessment and care planning skills.Excellent documentation and organizational abilities.Proficient with electronic health records (EHR) and basic computer systems.Valid driver’s license and reliable transportation.Current CPR/BLS certification.Ability to pass background check and drug screening per agency and VA requirements.Preferred ExperiencePrevious experience in private-duty home care or non-medical home care agencies.Knowledge of VA Community Care Network (Optum/TriWest) processes.Experience participating in regulatory surveys or audits.Case management or supervisory nursing experience.Physical & Work RequirementsAbility to travel to client homes throughout the service area.Ability to lift up to 25 pounds as needed.Ability to stand, walk, bend, and sit for extended periods.Comfortable working independently in community settings.Schedule & CompensationPRN / Per Diem – Flexible scheduling based on agency and client needs.Competitive compensation based on experience.Mileage reimbursement (if applicable).Why Join Homewatch CareGivers?Flexible work scheduleSupportive local owners and leadership teamOpportunity to serve veterans and seniors in the communityMission-driven organization focused on high standards and compassionProfessional growth opportunitiesCompetitive pay schedule paid weekly
