Position Summary: The Pediatric Home Care Registered Nurse (RN) is responsible for providing skilled nursing care to patients aged 0-45 years, requiring professional nursing services. This role includes educating the patient, caregiver/family, and other nursing team members, always adhering to the physician's orders and the established care plan, under supervision. Hiring Requirements: A graduate of an approved school of professional nursing. A current nursing license in good standing as a Registered Nurse in the state where you practice. A minimum of six (6) months recent, verifiable RN nursing experience in pediatric acute care or a home health setting. At least one (1) year is preferred. If caring for patients over age 20, six (6) months of recent, verifiable RN nursing experience in acute care or home health is required, with one (1) year preferred. Professional demeanor and effective oral and written communication skills. Ability to read, write, and communicate effectively in English. A valid state driver's license and automobile liability insurance are mandatory. Seat belts must be worn at all times while driving. Essential Responsibilities: Understand the unique responses of patients to illnesses and create an environment conducive to optimal growth and development. Provide education and assistance to families/caregivers to promote independence in caring for their child/adult. Deliver direct patient care, evaluate outcomes, collaborate with other professionals as needed, and adjust care processes to ensure the best patient outcomes. Participate in developing the Nursing Care Plan by assessing patient needs comprehensively, including safety, nutrition, medication, and environmental factors in collaboration with the Clinical Supervisor, patient, family, and physician. Regularly reevaluate patient needs based on assessments and responses to care, promptly communicating any changes to the physician and Clinical Supervisor. Respect patient and family rights and maintain confidentiality of all patient-related information. Ensure adequate supply stock and proper functioning of equipment. Complete documentation according to Thrive Skilled Pediatric Care's policies. Provide timely updates on all patients to the Clinical Supervisor to facilitate patient safety and secure ongoing care approvals. Participate in orientation and training of new employees as a preceptor when needed. Enhance professional knowledge through ongoing education, participating in staff meetings, conferences, and professional organizations. Support Quality Improvement initiatives and contribute to projects/studies. Serve as a patient advocate. Represent Thrive Skilled Pediatric Care by following departmental policies and standards related to quality improvement, safety, and infection control. Perform other duties as assigned. Travel Requirements: Travel to patient homes and office locations is necessary. Essential Physical Demands: Risk Category: May be exposed to blood and/or body fluids during job duties. Potential exposure to temperature extremes and humidity when traveling and in patient homes. May encounter dust, gas, fumes, and odors during travel. Physical Demands: Must be able to travel to office locations, other branches, and patient homes. Manual dexterity required for operating patient care equipment. Capability to use office equipment, including computers, printers, and copiers. Visual acuity needed to review and evaluate documentation. Ability to frequently lift, push, or carry up to 50 pounds.
