We pride ourselves on hiring professionals who understand the dynamics of Home Health.Qualifications:Graduate from a two-year college level program approved by the American Physical Therapy AssociationCurrently licensed in the state of TexasTwo year of experience in Home Health preferredResponsibilities:Understands and adheres to established policies and proceduresProvides physician prescribed physical therapy under a plan of care established by a PTImproves or minimizes residual physical disabilities of patientsReturns the individual to optimum and productive level within patient’s capabilitiesPerforms all skilled procedures as ordered by physician and according to the plan of care established by a PTParticipates with all other health care personnel in patient care planningTakes initial history and perform initial evaluationConsults with PT regarding change in treatmentInstructs patients and family/significant others in home programs and activities of daily livingParticipates in mandatory in-service programs and presents in-service programs as assignedParticipates in performance improvement activities as assignedAttends all patient care conference as scheduledPrepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per Agency
