Occupational Therapist - Consultation Services

About Us: At Community Health and Wellness (CHAW), we are committed to improving the lives of individuals with developmental disabilities through compassionate support and expert consultation. Position Overview: We are on the lookout for a dedicated Occupational Therapist (OT) to join our compassionate team and provide valuable Therapeutic Consultation for Virginia's Developmental Disabilities Waivers (FIS/CL). In this rewarding role, you will deliver consultative services, assessing client needs, crafting tailored OT support plans, training family members and caregivers, and monitoring client progress. Please note that this position does not entail ongoing direct therapy. Key Responsibilities: Conduct thorough OT assessments and observations to understand client needs. Develop detailed written OT support plans that provide personalized strategies and adaptations. Educate family members, caregivers, and Direct Support Professionals (DSPs) on effective intervention techniques. Engage in annual Individual Support Plan (ISP) meetings and collaborative team reviews. Ensure complete and accurate Medicaid-compliant documentation including notes, training logs, and progress summaries. Qualifications: Must hold a valid Occupational Therapist license in Virginia. Experience working with individuals with intellectual and developmental disabilities (ID/DD) is highly desirable. Schedule & Modality: Sessions are typically 1 hour in duration and can be conducted in-person or via HIPAA-compliant teleconsultation. The frequency of sessions will be based on assessed needs and ISP authorizations.
