Home Health Aide (HHA) As a dedicated Home Health Aide, you will provide essential direct patient care under the guidance of a registered nurse and in accordance with the Patient Care Plan (POC). Your role is vital in enhancing patient comfort and independence. You will assist patients with self-administered medications by ensuring their safety and understanding. This includes opening medication containers, reading labels, verifying dosages, and confirming that medications are taken as prescribed. Your responsibilities will include accurately measuring and reporting vital signs such as temperature, pulse, and blood pressure. You will recognize and report any abnormalities promptly to your supervisor. Maintain a high standard of personal hygiene for patients through direct assistance or supervision in their daily routines including bathing, grooming, skin care, oral care, and more. Support patients during meal times by assisting with feeding and monitoring food and fluid intake, while also implementing basic nutritional principles. Help patients with toileting needs, including the use of bedpans and catheter management, ensuring dignity and comfort. Facilitate patient mobility and exercise routines as outlined in the care plan, ensuring safe transfers, positioning, and ambulation. It's essential to document any changes in patient conditions and interactions thoroughly and legibly, contributing to the overall continuity of care. Uphold infection control practices to maintain a clean, safe, and comfortable environment for all patients. In addition, you may assist with light housekeeping tasks such as laundry and meal preparation, as long as these tasks are in alignment with your patient’s needs and physician directives. Required Experience/Skills: High school diploma or equivalent Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months Compliance with state certification requirements A valid driver’s license, auto insurance, and reliable transportation Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course for a Medicare-certified agency At least one year of experience within the last 24 months as a Home Health Aide or Certified Nursing Assistant in various care settings.
