RN CASE MANAGER - HOME HEALTH | SUMMARY AND HIGHLIGHTS: The RN Case Manager will coordinate care for high-risk members within the Charlottesville and Staunton territories. This role focuses on performing comprehensive assessments, developing individualized care plans, and collaborating with a multidisciplinary team to ensure clinical and fiscal outcomes are met. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needsBenefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insuranceA 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN CASE MANAGER - HOME HEALTH | RESPONSIBILITIES: Assessment & Care Planning: Perform comprehensive assessments of high-risk members and collaborate with primary care providers to implement individualized care plans.Care Coordination: Act as a liaison between physicians, social workers, discharge planners, and claims professionals to transition patients to the appropriate level of care.Advocacy & Support: Identify non-medical barriers to treatment (e.g., transportation) and put supports in place to ensure compliance.Clinical Excellence: Identify opportunities for health promotion and illness prevention, intervening quickly to minimize poor outcomes.Documentation: Maintain detailed records of clinical, functional, and fiscal outcomes using Homecare Homebase (HCHB). RN CASE MANAGER - HOME HEALTH | REQUIREMENTS: Education: Associate’s Degree in Nursing (ADN) or related field.Attendance: Commitment to mandatory attendance during the first 90 days.Licensure: Current, unrestricted Multi-State Registered Nurse (RN) license.Required Experience: Proficiency with OASIS and Homecare Homebase (HCHB) is mandatory.Certifications: Active CPR/BLS certification and Case Management Certification (CCM preferred).Health Screenings: Valid TB test and Wound/Competency testing required.
