We have an exciting opportunity for a Congestive Heart Failure specialist to become part of our team at St. Vincent's Medical Center in Bridgeport, CT. Why Consider This Opportunity? Autonomously manage a panel of patients recovering from congestive heart failure and those facing chronic acute exacerbations in our outpatient ambulatory clinic. Engage in remote monitoring for heart failure patients and provide care for those with Left Ventricular Assist Devices (LVAD). Collaborate closely with cardiologists while overseeing and referring patients for comorbid conditions. Enjoy competitive per diem compensation with available shifts Monday to Friday, from 8 AM to 4 PM. Work in an autonomous setting, managing complex patients and enhancing their health outcomes. Take your career to new heights by applying today! Qualifications: Must be eligible for licensure as an Advanced Practice Registered Nurse (APRN) in Connecticut. Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult Gerontology Primary Care Nurse Practitioner (AGPCNP), Acute Care Nurse Practitioner (ACNP), or Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) is required. Previous experience as an Advanced Practitioner is essential, and experience in Congestive Heart Failure management is highly preferred.
