Licensed Practical Nurse (LPN) Full-Time - Salisbury, MD Salary Range: $50,000-$70,000 annually Now Hiring for a 4-Day, 36-hour Work Week (Monday-Thursday!) Position Summary HealthPort is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our care team. In this role, you will support providers during office visits, deliver skilled nursing services, and ensure high-quality, patient-centered care. You will also be responsible for accurate and timely documentation within our electronic medical record (EMR) system, contributing to a collaborative and efficient clinical environment. Key Responsibilities Maintain strict confidentiality in accordance with HIPAA, CARF accreditation standards, and Maryland state regulations. Utilize the EMR to enter, track, and complete all clinical services. Participate fully in all required agency training programs. Coordinate specialty referrals and collaborate with external providers to ensure continuity of care. Complete prior authorizations for services and medications; work closely with pharmacies, pharmaceutical representatives, and service vendors. Administer prescribed medications, including injections, under the supervision of a prescriber or registered nurse. Collect and process laboratory specimens accurately and efficiently. Complete all client treatment documentation and share necessary information promptly with appropriate care team members and authorized external partners. Assist providers during office visits and deliver scheduled skilled nursing services. Escort patients to exam rooms; conduct patient interviews; measure and record vital signs. Monitor the nurse line to triage patient needs and coordinate care with the interdisciplinary team, including pharmacy support. Maintain clean, organized, and fully stocked exam rooms; prepare instruments and sterilization materials; properly dispose of contaminated or biohazardous supplies. Manage telephone messages and provide timely follow-up to patient, provider, and pharmacy inquiries. Collaborate with treatment teams to support rehabilitation and holistic patient needs. Provide health education and training as requested. Perform additional duties as assigned by the supervisor. Qualifications & Experience Current license issued by the Maryland Board of Nursing. Strong teamwork skills with the ability to collaborate effectively across disciplines. Excellent organizational and communication skills. Availability to work occasional evenings as needed. Current CPR certification. Privileged to perform all required duties, including treatment coordination. Why Join HealthPort?
