Nurse Practitioner-Primary Care

Position Summary The organization is seeking a Primary Care Nurse Practitioner (APRN) to join its established outpatient practice in New Haven, Connecticut. Candidates with a background in Family Medicine or Internal Medicine-focused primary care will be considered. This is a full-time, permanent outpatient position providing comprehensive primary care services to adult and geriatric patients. Essential Responsibilities Provide comprehensive primary care services, including preventive care, chronic disease management, and acute visits Diagnose, treat, and manage common medical conditions across the adult patient population Perform annual physicals, wellness visits, and health risk assessments Order, review, and interpret diagnostic tests and laboratory results Collaborate with physicians, specialists, and interdisciplinary care teams Maintain accurate and timely documentation in the EMR Participate in quality improvement initiatives and evidence-based care delivery Educate patients on disease prevention, treatment plans, and healthy lifestyle choices Qualifications Education: Master's or Doctoral degree in Nursing from an accredited program Licensure: Active Connecticut APRN license (or eligibility) Certification: Board Certified as FNP or Adult/Gerontology NP Training Focus: Family Medicine or Internal Medicine-based primary care Experience: All experience levels welcome; new graduates encouraged to apply Compensation & Work Environment Competitive compensation package based on experience Outpatient-only practice setting Predictable schedule supporting work-life balance Strong clinical and administrative support structure Thank you, and I look forward to connecting soon!
