Key Responsibilities: Collaborate with therapists, nurses, and other healthcare professionals to create and implement comprehensive treatment plans tailored to patients with varied rehabilitation needs. Conduct thorough patient evaluations, including reviewing medical histories, performing physical assessments, and executing diagnostic evaluations to establish clear rehabilitation objectives and strategies. Monitor patient progress consistently and modify treatment plans as required to enhance functional outcomes and support patient independence. Participate actively in multidisciplinary team meetings and case discussions to continuously improve clinical knowledge and skills. Maintain timely and accurate documentation of initial evaluations, treatment plans, progress notes, and discharge summaries within electronic medical record systems. Educate and counsel patients and their families about the rehabilitation process, expected outcomes, and post-discharge care strategies. Qualifications: Valid state nursing license. Current Controlled Substance License (required upon start date). Current DEA License (required upon start date). ANCC Certificate/AANP Certificate. A genuine passion for Physiatry/Physical Medicine and Rehabilitation. Orthopedic experience (preferred). Experience in Physiatry (preferred). Skilled nursing experience (preferred). Licensure/Certification: Certified Nurse Practitioner (Required). Compensation & Benefits: Available positions for full-time and part-time roles.
