Your Impact as a Nurse Practitioner In this fulfilling role, you will conduct detailed health assessments during visits, lasting up to one hour. You will evaluate a member's health status, medical history, medication adherence, and social circumstances, providing crucial insights for quality care. Your responsibilities will encompass: Conducting comprehensive assessments for adults and geriatrics, including analyzing medical histories, formulating diagnosis and treatment plans, health education, and referrals. Administering and ordering necessary point-of-care and laboratory tests. Collaborating with healthcare teams, including physicians, case managers, social workers, and family members. Partnering with Primary Care Physicians for patient education and follow-up care. Delivering services at members' homes or Skilled Nursing Facilities. Essential Qualifications Master's Degree or equivalent training and completion of NP licensure. Valid NP licensure in the practicing state, including prescriptive authority or the capability to obtain it. Board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty from recognized boards. Current BLS, ACLS, or CPR certification. A minimum of one year of experience as a Nurse Practitioner. Ability to travel 80-90% of the time, including overnight travel and weekends across assigned and neighboring states. Valid state driver's license and auto insurance per state regulations. Proficiency in computer skills and familiarity with medical record software. Strong verbal and written communication abilities.
