PTO Free meal on each shift Reportsto:Director of Nursing JobDescription: As an RN or LPN in a Skilled Nursing Facility, you will beresponsible for providing skilled nursing care and oversight to residents withcomplex medical conditions, chronic illnesses, or disabilities. You willcollaborate with other healthcare professionals, including physicians,therapists, and nursing assistants, to assess, plan, implement and evaluatecare for residents. KeyResponsibilities: Assess residents' health status and develop and implementindividualized care plans that address their unique needs and preferences. Administer medications and treatments as ordered by physiciansand ensure residents' compliance with prescribed regimens. Monitor and document residents' vital signs, symptoms, andresponse to treatments. Collaborate with interdisciplinary teams to manage residents'medical needs, including wound care, physical therapy, and other therapies. Coordinate with families and community resources to promoteresidents' physical, emotional, and spiritual well-being. Participate in quality improvement programs, infectioncontrol, and risk management initiatives. Provide support and guidance to nursing assistants and otherstaff members, as needed. Perform other duties as assigned. Qualifications: Active Registered Nurse (RN) license or Licensed Practical Nurse in the state of Ohio Knowledge of nursing principles, practices, and techniques, aswell as current trends and developments in the field. Ability to assess and interpret data and communicateeffectively with residents, families, and other healthcare providers. Strong interpersonal skills and the ability to workcollaboratively with interdisciplinary teams. Basic computer skills to maintain electronic medical recordsand documentation. Ability to work flexible schedules, including weekends andholidays. PhysicalDemands: TheRN or LPN in a skilled nursing facility position requires the ability to performessential functions of the job, including the ability to stand, sit, walk,bend, lift, and move intermittently throughout the workday. The position alsorequires the ability to communicate clearly and effectively with residents,families, and healthcare providers. Note: Thisjob description is not intended to be all-inclusive.
