A Typical Day for a Registered Nurse Case Manager with WesleyLife At Home Hospice: As a Registered Nurse Case Manager with WesleyLife at Home Hospice, each day begins with purpose-the opportunity to bring comfort, dignity, and compassionate support to patients and families navigating one of life's most meaningful journeys. Your role blends clinical excellence with relationship-centered care, allowing you to make a profound difference in the lives of those you serve. During you're visits as a Registered Nurse Case Manager you will: Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall well-being Manage pain and other symptoms using evidence-based hospice practices Coordinate medications, treatments, and supplies to support patient needs Prepare and update individualized care plans in collaboration with the interdisciplinary team Your presence brings reassurance and stability during a vulnerable time. Being a Steady Support for Families Family members often look to you for guidance, clarity, and emotional support. You'll: Provide education on what to expect throughout the hospice journey Teach family caregivers how to provide safe, comfort-focused care Offer a listening ear and calm presence during moments of uncertainty You become an essential source of comfort-not just for patients, but for loved ones as well. Collaboration That Elevates Care As a case manager, you serve as the central point of coordination for each patient's care. You'll collaborate regularly with: Hospice aides Social workers Spiritual care providers Physicians Together, you ensure each patient receives holistic, person-directed care aligned with their wishes. Open Shifts Available: Full-Time Monday-Friday, Back-up on-call Weekday and weekends Qualifications: Valid, unrestricted Iowa driver's license. Current vehicle insurance. Current license as a registered nurse in the state of Iowa. Two years of full-time nursing experience required, with one year of hospice experience preferred.
