Job Title: Case Coordinator / Manager Exp with Adult Autism Waiver (AAW) Location: Harrisburg, PA 17120 Duration: Contract 6 + Months Job Mode: On-Site Pay rate: $36.21/hour on w2 without benefits Shift hours: Monday through Friday Day shiftJob Description: Monitors and provides consultative services to Supports Coordinators and providers for the Adult Autism Waiver (AAW) to assure compliance with state regulations, policies and procedures. Evaluates the quality of AAW services and supports through on site visits and review of records. Interviews and meets with AAW consumers to ensure needs are being addressed. Conducts annual quality assurance monitoring of AAW providers and participates in the development, coordination and implementation of corrective action plans. Reviews, approves and monitors Individual Support Plans (ISP). Provides general case management services for AAW participants. Makes recommendations on how to improve programs and service delivery. Determines final non-financial eligibility for the AAW. Meets with service providers to explain waiver services, policies, and regulations and also work with providers to ensure compliance with the AAW. Minimum experience and training: A bachelor's degree is preferred, but not required. A combination of education, experience and training, which may include experience working in a public or private human service program with at least 12 college level credits of education in a social science or human services, education, administrative experience such as case management, service delivery, data collection, or program monitoring/quality assurance experience will be considered in lieu of a bachelor's degree . Intermediate Computer skills to include, but not limited, to Microsoft Office Products.
