Your key responsibilities will include: Performing patient visits and accurately documenting care provided Consulting with physicians regarding patient care needs Conducting precise OASIS assessments Maintaining up-to-date medication profiles and ensuring timely communication of lab values to physicians Participating in weekly case conferences and monthly case manager meetings Qualifications to Join Us To be a successful candidate for this position, you should meet the following qualifications: Current licensure as a Registered Nurse (RN) in your state A minimum of one year of clinical experience is preferred Demonstrated knowledge and skills in current nursing practices Valid state driver's license and automobile liability insurance Additional Information At Enhabit Home Health & Hospice, we proudly uphold the values of diversity and equality.
