Key Responsibilities: Work collaboratively with therapists, nurses, and other healthcare professionals to design and implement holistic treatment plans for patients with varied rehabilitation needs. Perform comprehensive patient assessments, including reviewing medical histories, executing physical examinations, and conducting diagnostic evaluations to set precise rehabilitation goals and strategies. Closely monitor patient progress and modify treatment plans as needed to enhance functional outcomes and support patient independence. Actively engage in multidisciplinary team meetings, case discussions, and educational initiatives to continually enrich clinical knowledge and skills. Ensure accurate and prompt documentation of initial evaluations, treatment plans, progress notes, and discharge summaries within electronic medical record systems. Educate and counsel patients and their families about the rehabilitation process, expected results, and post-discharge care strategies. Qualifications: Active state licensure to practice nursing. Current Controlled Substance License (required upon start date). Current DEA License (required upon start date). ANCC Certificate/AANP Certificate. Genuine interest in Physiatry/Physical Medicine and Rehabilitation. Orthopedic Experience (preferred). Experience in Physiatry (preferred). Skilled Nursing Experience (preferred). Licensure/Certification: Certified Nurse Practitioner (Required). Compensation & Benefits: Full-time and part-time positions available. Competitive compensation package with performance bonuses.
