Hospice Social Worker

University Home Care Inc is seeking a compassionate and dedicated Hospice Social Worker to join our team in Livonia, MI. In this role, you will provide comprehensive psychosocial support to patients and their families, ensuring they receive the highest quality of care during hospice services. You will collaborate with interdisciplinary teams to address emotional, social, and practical needs, helping patients navigate end-of-life challenges with dignity and respect. Responsibilities Conduct psychosocial assessments of patients and families to identify needs and develop appropriate care plans. Provide counseling and emotional support to patients and families coping with terminal illness. Coordinate community resources and referrals to assist with patient and family needs. Facilitate family meetings and interdisciplinary team conferences. Advocate for patient rights and ensure care aligns with patients' goals and wishes. Maintain accurate documentation and comply with regulatory requirements. Support bereavement services for families after the patient's passing. Requirements Master's degree in Social Work (MSW) from an accredited institution. Licensed Clinical Social Worker (LCSW) or eligibility for licensure in Michigan. Experience in hospice, palliative care, or related healthcare setting preferred. Strong assessment, counseling, and crisis intervention skills. Ability to work collaboratively within a multidisciplinary team. Excellent communication and interpersonal skills. Valid driver's license and reliable transportation.
