The American Heart Association is excited to offer a role as a Community Connector with our Rural at Heart team, based at the Porter County Health Department site in Valparaiso, IN. This position focuses on improving cardiovascular health in rural communities by collaborating with clinical and community partners. As a Community Connector, you will work on key initiatives including blood pressure management, enhancing community readiness, addressing chronic illnesses, and guiding community members in navigating healthcare barriers. This is a full-time (37.5 hours per week), benefits-eligible, grant-funded position, with funding currently secured through September 4, 2026. There may be flexibility for part-time arrangements. Responsibilities Implement evidence-based practices for blood pressure control in clinical and community settings, providing necessary education. Support local hosts and community partners in enhancing health outcomes. Assist in executing nutrition security initiatives like screening, referral, and access to healthy food programs. Promote community readiness through the development of Cardiac Emergency Response Plans and conducting hands-only CPR training. Deliver health education individually and in group settings covering a range of cardiovascular health topics. Foster relationships and effectively collaborate with community stakeholders. Recruit, train, and coordinate volunteers and community partners to achieve community goals and increase engagement. Communicate program objectives and offer guidance to host sites and partner organizations. Engage regularly with host sites, American Heart Association supervisors, community members, and partners to deliver education and connect them to resources. Participate in training events and organizational/community meetings, both virtually and in person. Qualifications High school diploma or equivalent; coursework in public health, health promotion, or health sciences preferred. Strong communication and organizational skills. Experience working independently and collaboratively. Passion for advancing rural health outcomes. Intermediate proficiency in Microsoft Outlook and Word. Proven ability to manage projects and events effectively. Experience collaborating with a diverse range of stakeholders. Flexible to travel locally (20 - 25% of time) as required; reliable transportation is necessary. Willingness to attend training events, possibly including one or two week-long sessions, and participate in community meetings. Ability to lift and move larger items when necessary, requesting assistance as needed. Access to reliable internet when working remotely. Compensation & Benefits The expected pay ranges from $18.20 to $22.20 per hour, based on experience, with potential geographic adjustments.
