Job Description Job Description The Family Nurse Practitioner / Physician Assistant will provide comprehensive primary care services to patients across the lifespan in an outpatient setting, with a strong emphasis on caring for Vietnamese speaking patients. The clinician serves Vietnamese Black and Latinx communities in New Orleans East. This role will manage acute and chronic conditions, and work within an interdisciplinary, team based care. This role is ideal for an advanced practice clinician who wants to combine solid clinical practice with meaningful community impact. Essential Duties and Responsibilities Direct Patient Care Provide evaluation, diagnosis, and treatment for acute and chronic conditions for pediatric, adult, and geriatric patients within licensure and scope of practice. Perform comprehensive histories and physical exams, develop differential diagnoses, and create evidence based treatment plans. Conduct patient visits for a multicultural patient base and use interpreters and translation language services. Prescribe medications, order and interpret laboratory and diagnostic studies, and provide appropriate follow up care. Perform routine in office procedures consistent with primary care practice and training. Chronic Disease & Preventive Care Manage common chronic diseases (e.g., diabetes, hypertension, hyperlipidemia, COPD/asthma, hepatitis, depression/anxiety). Promote prevention and wellness, including immunizations, cancer screenings, and annual wellness visits. Provide culturally appropriate counseling on lifestyle modification, medication adherence, and self management strategies. Care Coordination & Team Based Care Collaborate with physicians, nurses, behavioral health providers, MAs, care coordinators and community health workers to deliver integrated care. Participate in daily or regular huddles, case conferences, and warm handoffs with behavioral health and enabling services. Work with team to ensure coordination of referrals to specialists, hospitals, and community resources and ensure timely follow up. Support transitions of care from hospital or emergency department back into the primary care setting. Documentation & Quality Document all encounters accurately, completely, and timely in the Athena, NOELA's electronic health record (EHR), in accordance with payer, and regulatory requirements. Follow clinical guidelines, standing orders, and care pathways established by the organization. Participate in quality improvement initiatives, including UDS and other performance measures, care gap closure, and panel management. Adhere to HRSA, state, and other regulatory and compliance standards relevant to FQHC practice. Community & Cultural Responsiveness Provide linguistically and culturally appropriate services for Vietnamese and other limited English proficiency patients. Assist in developing and/or reviewing patient education materials in Vietnamese. Participate in occasional community outreach activities, health fairs, and educational events in New Orleans East, which may sometimes occur in evenings or on weekends. Minimum Qualifications For FNP: Master's or Doctoral degree in Nursing and current, unrestricted Family Nurse Practitioner license in Louisiana. For PA: Graduate of an accredited Physician Assistant program and current, unrestricted PA license in Louisiana. Current national certification (AANP/ANCC for FNP; NCCPA for PA). Valid DEA registration (or eligibility to obtain) for practice in Louisiana. Current BLS certification. Demonstrated Vietnamese language skills (conversational or fluent) sufficient to communicate directly with patients and families. Experience in primary care, family medicine, internal medicine, or community health (strong new graduates with relevant background considered). Preferred Qualifications Prior experience in an FQHC, community health center, or similar safety net environment. Experience working with a multicultural community. Familiarity with team based care, PCMH, and value based care models. Experience with chronic disease management programs and quality metrics. Proficiency with electronic health records (e.g., Epic, eClinicalWorks, or similar). Core Competencies Commitment to health equity, social justice, and patient centered care. Strong interpersonal and communication skills with patients, families, and colleagues. Ability to work effectively in a fast paced, resource limited outpatient setting. Cultural humility and respect for the values, traditions, and experiences of the Vietnamese and broader New Orleans East communities. Work Schedule and Environment Primarily weekday clinic hours with potential for extended hours or occasional evenings/weekends for community events or expanded access. Outpatient clinic setting with integrated behavioral health and care management support. Compensation and Benefits Competitive salary commensurate with experience.
