Job Description

Reports to: Director of Nursing Join our compassionate team as a Pediatric Home Health Licensed Practical Nurse (LPN), where you will play a vital role in delivering exceptional care to children in the comfort of their homes. This position entails assessing, planning, implementing, and evaluating comprehensive patient care while effectively coordinating teaching for patients and their families. Key Responsibilities: Provide skilled nursing care to assigned pediatric patients. Assess children's physical and emotional health, home environments, and family support systems. Inform patients and families about their rights and care options. Evaluate ongoing patient status, implement care plans, and communicate with physicians and clinical staff. Perform nursing procedures as directed by physicians, including medication and IV administration. Supervise Home Health Aides to ensure quality care. Assist patients with their daily activities, fostering independence and self-sufficiency. Maintain accurate clinical records and reports for each patient. Participate in on-call rotations as needed. Complete additional duties as assigned by management. Supervisory Responsibilities: Guide and evaluate patient care, including coordination with team members. Involves hiring, training, performance evaluations, and addressing staff concerns. Qualifications: Current LPN license in good standing. Graduate of an accredited nursing program. One year of nursing experience preferred but not required. Skills and Abilities: Exceptional nursing, observation, communication, and leadership skills. Familiarity with home health care and regulatory guidelines. Good physical and mental health is essential. Empathetic and patient demeanor with a caring attitude. Able to resolve practical issues and interpret diverse instructions effectively. Physical Requirements: Moderate physical activity, including driving to patient locations and assisting in mobility activities. Regularly required to use hands, talk, and hear; frequently stand, walk, and bend. Ability to lift over 100 pounds occasionally. Vision must include close vision, color vision, and depth perception.
