Home Health Registered Nurse

Position Summary: We are seeking a compassionate and skilled Registered Nurse (RN) to provide exceptional nursing services to our clients in accordance with the state Nurse Practice Act and agency policies. You will utilize your comprehensive nursing knowledge to implement personalized care plans under medical guidance. Reporting To: Clinical Supervisor Qualifications: Graduate of an accredited nursing program; BSN preferred. Current RN license in the state of operation. Excellent communication and interpersonal abilities. Current CPR certification. Licensed driver with an insured automobile that meets state requirements. Essential Functions / Areas of Accountability: Take responsibility for nursing practices in compliance with state regulations and home health standards. Conduct thorough physical assessments and comprehensive assessments for client admissions, as directed. Offer guidance to Licensed Practical/Vocational Nurses (LPN/LVNs) and/or Home Health Aides (HHAs) when applicable. Promptly communicate any changes in client conditions to the overseeing medical practitioner and care team. Contribute to the development and adjustments of client care plans. Demonstrate proficiency in technical nursing skills within your legal scope of practice. Provide nursing care that respects clients' individual, cultural, and spiritual differences. Adhere to agency policies, payor guidelines, and accreditation requirements. Initiate actions aimed at enhancing client care quality and preventing negative outcomes, with an emphasis on safety and infection control. Uphold confidentiality across all job functions. Engage in ongoing professional development. Delegate tasks to LPN/LVNs and/or HHAs as necessary based on legal scope of practice. Perform additional duties as needed.
