Given the start date, for credentialing, client is requiring Washington State license to be in hand at time of present Reason for opening: Gap in coverage Type of privileges (clinic/hospital/both): Both (Provider won t be working in clinic, but this is indicated as both for privileging only) Setting: Inpatient, Outpatient or Both? Inpatient If Inpatient, # of beds: Facility has about 400 beds Call requirement: Call shifts Rounding, admissions, or none? Rounding and admissions. Will also help cover ED and inpatient consults, as well as other emergency surgeries Patient demographics: Adults to geriatric Average patients per day: 5-20 Clinical Activity Requirement: Applicant must provide documentation (case list) of provision of clinical services (100 procedures) representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed. Required Procedures: All general vascular and endo vascular Will provider work solo? If not, other providers specialties working with them: No, will be working with trauma, acute care, general surgery and other departments Licensed Candidate: Yes/No: Yes, Washington license required to be in hand at time of present Required certifications: Board Cert, other certs required by MSO EMR: EPIC Schedule Details 04/06/26-04/13/26 7am-7am (Orientation will be done the morning of 04/06/26) 24-hour call shifts 7am-7am, Monday to Monday morning This is call only, no assigned clinic work. They will get paged for consults for the hospital. Monday-Friday call volume average: 6-8 hours Saturday-Sunday call volume average: 6-8 hours
