Job specification Candidate requirements Job specification Conduct in-home assessments to monitor patient safety, identify health changes, and provide medication reviews and clinical coaching. Candidate requirements Active Registered Nurse (RN) License in the state of Massachusetts. Minimum of 2 years of recent direct care experience with elders or adults with disabilities. Ability to travel locally; must possess a valid driver's license and auto insurance. Prior experience in case management, care planning, and clinical assessment-especially in eldercare, complex medical cases, or disability support.
