Pharmacist Boulder Community Health, Boulder, CO 80303 $57.86 - $86.79 an hour - Half-Time, 18 hours per week +$8.00/hr for night shifts. +$2.50/hr for evening and weekend shifts. Boulder Community Health is seeking a pharmacist responsible for ensuring the safe, effective, and cost-efficient use of medication, providing consultation and education to healhtcare professionals and patients, and overseeing medication practices for individuals across all age groups. They will lead pharmacy services in inpatient and outpatient settings, supervise support personnel, and collaborate with a multidisciplinary team to implement patient care. The Pharmacist evaluates medication orders, reviews cost-effective utilization, and ensures compliance with regulatory and quality standards. They will also engage in continous learning, contribute to hospital comittees, and stay current with developments in pharmacy practice. Graduate of an accredited residency program preferred. •Current Licensed Pharmacist in the State of Colorado required. •Board certification preferred. •BLS/ACLS preferred. Responsibilities •Interprets and fills medication orders for patients 0 through life span. •Maintains patient profiles. •Provides education and counseling to patients and their families about medications. •Monitors drug therapy for interactions, allergies, and appropriateness for patients of all ages. •Issues controlled substances and maintains records as required by law. •Provides education to nurses, physicians, and other staff as required. •Fills out patient prescriptions. •Prepares IVs using aseptic technique. •Understands and is able to operate the automated dispensing machines. •Participates in clinical programs provided by the Pharmacy.
