Position Summary: The Supervisor of Social Work & Care Coordination is responsible for providing direct supervision, day to day operational oversight and support to a diverse team of Care Coordinators and Social Workers. The Supervisor is also responsible for monitoring and evaluating team and program performance as defined by quality of care, compliance, financial and sustainability metrics under direction of the Manager of Social Work and Care Coordination. Compensation is dependent on skills and experience.Your contribution to the team includes: Primarily responsible for day-to-day operations of designated care coordination and Social Work team and activities under the direction of the Manager, including staffing, training, program activities and logistics. Ensure compliance with payer contracts and programmatic requirements of various projects. Travel to various locations including clinic, community, and home visits to provide care and support as needed. Responsible for hiring, performance management, talent development, employee relations, recognition, and training of staff by providing effective leadership. Ensure all work is completed in support and enhancement of the objectives and goals of the organization. Provide additional clinical training and support as needed. Actively partner with care team members to oversee the provision of advanced psychosocial support and Social Work services especially for situations involving domestic violence, homelessness, trauma, substance use, crisis intervention, complex family dynamics, newly arrived refugees and other complex social or behavioral health situations. Promote patient self-management, self-determination, and person-centered care. Provide case consultation as needed. Utilize population health and continuous process improvement strategies to mobilize teams and drive success as indicated by quality metrics, data analysis, financial analysis, and program goals. Evaluate, update, and revise processes to succeed on designated contracts and initiatives while maintaining staff productivity and workload balance. Be committed to a continuous learning environment where programmatic goals will shift based on the healthcare environment, requiring flexibility and prioritization. Assist staff with supporting patients towards improvement of medical, behavioral health and social outcomes by mobilizing teams and establishing relationships between primary medical, specialist, behavioral health, long term care services and community resources related to the patient's care plan goals. Ensure staff maintain appropriate patient panel sizes. Partner with leadership to provide cohesive oversight, support, and development of staff. If fully licensed, provide Social Work clinical supervision for staff seeking licensure. Encourage and maintain positive work environment centered around teamwork, collaboration, and accountability. Adeptly navigate sensitive conversations with staff including development plans, coaching, performance, and productivity with emotional intelligence that nurtures self-awareness and growth. Provide coaching and guidance to staff to ensure they maintain professional relationships and boundaries as they support patients, families or caregivers with empathy, compassion and cultural congruence while maintaining respect for confidentiality, privacy, and mandated reporting. Collect, analyze, and act on data regarding the performance of designated programs, including enrollment, capacity metrics and financial status to inform and expand services as needed. Implement and refine procedures, workflows, and measures to align with contract deliverables and business needs. Under direction of the Manager, lead teams and facilitate communication between staff, clinic leadership, and care teams aimed at improving understanding of Social Work and care coordination services including metrics and program alignment. Ensure reasonable standardization of workflows and program implementation across clinic and other sites. Support and implement care model or staffing changes as needed. In collaboration with Manager, maintain clinical oversight, evaluation and implementation of various screening tools, assessments, and interventions such as PRAPARE or other Social Determinant of Health screening, KATZ, GAD-7, PHQ-9, motivational interviewing, psychosocial assessments, and safety planning with patients. Provide staff training, coaching and support as needed. Establish new relationships with diverse care team members, including community partners and external organizations (such as MCOs) to promote health, wellness, effective coping and disease management of populations. Ensure effective communication exists between staff, care team members, coworkers, patients, and support systems. Accountable for ensuring appropriate identification of patients for eligibility for Social Work, care coordination or support. Assign to designated staff accordingly as needed. Provide direct support to patients as needed to ensure accurate and timely communication or documentation occurs based on staffing and program fluctuation. Oversee appropriate utilization and documentation within program databases, documentation systems or electronic health records, including monitoring patient goal achievement. Provide technical support to assigned staff such as electronic system and resource navigation, facilitating computer or telecommunications and remote work support. Act as a change agent to address health disparities, increase health equity and advocate for person-centered approaches to care. Meet with team members, clinics, or patients in a variety of settings, including clinic, community, office, remote or in patient's home as needed. Maintain good attendance and punctuality. Demonstrate respectful, professional, and appropriate behavior that supports a team-oriented work environment. Continuously evaluate and improve processes and procedures to streamline and enhance office operations using the HealthPoint Lean Management System (HLMS) tools and processes. Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required. Provide direct Social Work and care coordination support to patients. Other duties as assigned by leadership. This position has supervisory responsibilities. Must have's you'll need to be successful: Master's Degree in Social Work (MSW) and at least three (3) years of Social Work experience; or equivalent combination of education and experience. Previous experience in a clinic or hospital setting, working with vulnerable populations, behavioral health or community health required. One of the following four credentials must be obtained within 90 days of hire and maintained for the duration of employment in accordance with WA Department of Health requirements: Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW); Licensed Social Work Associate Independent Clinical (LSWAIC) or Licensed Social Work Associate Advanced (LSWAA). Licensed Associate practitioners to be provided supervision by an approved licensed supervisor in the state of Washington to obtain full licensure. Valid Washington State Driver's License with an acceptable driving record determined by HealthPoint's insurance carrier. Possesses intermediate operating knowledge of computers. Intermediate experience with Word, Excel, and Outlook required, willing to learn OTTO or any other electronic telehealth platform. Electronic Health Record (EHR) experience required. Ability to read, analyze, and interpret common industry related journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write reports and articles for publication that conforms to prescribed style and format. Ability to effectively present information to top management, clients, external groups, and/or boards of directors. If you know about the following it's a plus: Two (2) years post-licensure experience preferred. Previous supervisory experience strongly preferred. Bilingual language proficiency preferred.
