Job Description Additional Job Information Sign-On Bonus Eligible $10,000 40 Hours per Week Monday - Friday 8:00 a.m. to 4:30 p.m., Flexible No weekends, holidays, or call Prefer wound care experience and/or certification Assists full-time wound care nurses in completing consults. Assists wound care nurse practitioner. Provides wound care education to new staff in patient care orientation. In charge of quarterly Prevalence studies. Benefits start on first day of employment Free employee parking Overview Job Summary: Coordinates the health care of select patient populations/designated service across the continuum of care. Responsible for coordination of patient contact, patient education, prepares patient/family for procedure/test in collaboration with the healthcare team. Job Responsibilities: Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs. Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice. Monitors care and data to optimize patient outcomes. Facilitate the education of patients and families. Acts as a resource to the health care team. Ensures actions meet and support the overall goals of the business.
