Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching participant, caregiver or other appropriate representatives/family to maintain participant support in the community.  EXPERIENCE EDUCATION AND CERTIFICATIONS: Advanced degree in Physical Therapy from an accredited school of Physical Therapy Current license to practice Physical Therapy in the state Current driver’s license and proof of auto insurance Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable. 1 year of experience working with a frail or elderly population preferred.
