PRIMARY FUNCTIONS: Collaborates with patients, physicians and other health professionals to ensure the responsible provision of drug therapy. Is knowledgeable about and complies with hospital and departmental policies, procedures, regulations and standards as related to the position. Works collaboratively with providers and other caregivers to optimize medication therapy (e.g. drug choice, dose, cost effectiveness, avoidance of drug duplications, interactions & ADR's). Is knowledgeable of and compliant with sterile compounding regulations (e.g. USP 797 and USP 800). Perform sterile compounding when needed. Reviews all provider orders that pertain to drug therapy for inpatients and Oncology/Infusion Center. Provides clinical services per department policies. These include consulting on antibiotic and anticoagulation therapy, renal impairment dose adjustments, parental nutrition formulations, IV to oral substitutions, therapeutic interchanges, and reviewing medication reconciliation. Maintains competencies necessary to efficiently use the information system and other technologies in pharmacy. Maintains records of drug distribution activities in compliance with Hospital policy, and state and federal law. Perform other duties as required. REPORTS TO: Director of Pharmacy Services SUPERVISES: Pharmacy Technician activities related to medication preparation and dispensing. Works cohesively with all pharmacy department personnel. EXTERNAL AND INTERNAL CONTACTS: All Hospital departments and medical staff, direct patient contact. Outside vendors, other medical professionals, professional organizations and the general public. CERTIFICATION/LICENSURE: Graduate of an accredited college of Pharmacy. Licensed as a Pharmacist in the State of New Hampshire. QUALIFICATIONS: Three (3) years experience in either community or hospital service preferred. Previous hospital experience desirable. Neat habits, capable of working under stress, willingness to work holidays and weekends. PHYSICAL DEMANDS: Regularly spends more than 50% of the working day standing or walking, frequently using stairways. Lifts, pushes or pulls light to average weight loads and occasionally pushes or pulls heavy loads. Visual acuity required as machines needing close visual attention are used. Moderate stress. WORK ENVIRONMENT: There is a potential for exposure to hazardous materials. HOURS WORKED: 40 hours per week. Required to work additional hours if circumstances warrant. Rotational shift for weekends and holidays.
