Case Manager RN

Job Summary We are seeking an experienced Registered Nurse Case Manager (RN CM) to join our team. In this role, you will coordinate patient care plans, evaluate medical necessity, support discharge planning, and facilitate collaboration across interdisciplinary teams. The ideal candidate is detail-oriented, compassionate, and skilled in resource management to ensure both quality outcomes and efficient patient throughput. Key Responsibilities Conduct comprehensive assessments of patients' medical and psychosocial needs Develop and document individualized case management plans of care Lead interdisciplinary team meetings and ensure collaboration for continuity of care Perform utilization management reviews and communicate with third-party payors Evaluate admissions for medical necessity and appropriate level of care Identify discharge needs and arrange necessary follow-up services and resources Act as a liaison between physicians, patients, families, and external providers Ensure compliance with regulatory requirements and quality initiatives Proactively track barriers to care and recommend process improvements Provide education, advocacy, and resources for patients and families Qualifications Required: Active FL RN license or Compact RN license (must obtain FL RN within 90 days of hire) 3 years of recent Case Management experience in an acute care setting OR 3 years of experience in Med/Surg, Telemetry, ICU, PCU, Neuro, or ED units Candidates with Case Management in home health or insurance may be considered with 3 years acute care (at least 1 year within the past 5 years) Preferred: BSN (Bachelor of Science in Nursing) Certification in Case Management, Utilization Review, or Nursing specialty Compensation & Incentives Competitive pay: $31.94/hr - $43.92/hr (based on experience) Shift differentials: Evening shift: $2.50/hr Weekend shift: $2.00/hr Evening weekend shifts stack for $4.50/hr total Sign-On Bonus: $10,000 (paid in installments over 2 years) Relocation Assistance: Available on a case-by-case basis Benefits Package We offer a comprehensive Total Rewards Program, including: Medical, Dental, and Vision coverage with prescription and behavioral health benefits Free Telemedicine & AirMed emergency transport services Life insurance, disability, flexible spending accounts, supplemental health plans 401(k) with 100% employer match (3-9% based on years of service) Employee Stock Purchase Program (10% discount) Paid Time Off, Paid Family Leave, and Disability coverage Tuition reimbursement, student loan assistance & certification support Fertility & family building benefits, adoption assistance Employee wellness & counseling programs Recognition & career growth opportunities Why Join Us?
