RN Field Supervisor - Per Diem - Rochester

Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team on a Per Diem basis!Schedule: 2 days per week 8:30am-4:30pmDuties & Responsibilities Provide assessment of newly referred patients and assess health needs in home care setting. Work with patient to create a patient centered plan of care. Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family. Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services. Provide supervision and training of PCA/HHA staff. Qualifications NYS Licensed Registered Professional Nurse. Minimum of Associate's Degree required. One year Nursing experience required. Home care experience a plus.
