Field Based Services Team Lead II (licensed) (609) Visalia CA The Field Based Services Lead plays a key leadership role within a multi-disciplinary behavioral health team, providing direct clinical support to team members, as well as providing mental health services to our target population. Under the general direction of the Clinical Manager and with substantial latitude for independent judgment, the Field Based Services Team Lead is responsible for delivering mental health services to individuals in crisis, conducting risk assessments, biopsychosocial assessments, treatment planning coordination, MSE exams, administer diagnosis of SMI disorders, determining levels of care, coordinate with law enforcement, families, and other agencies to ensure client safety and continuity of care. In the absence of the Clinical Manager, this role assumes selected administrative, procedural, and clinical responsibilities to ensure team operations run smoothly. How will I contribute Provide direct mental health crisis intervention and assessment services across the county. Independently conduct biopsychosocial assessments, mental status exams, determine level of care, identify crisis situations and complete risk assessments, safety plans, and provide individual therapy and psychosocial rehabilitative services to a complex caseload of clients. Develop and implement treatment plans, using trauma-informed, client-centered, culturally responsive, harm reduction and recovery-based approach, that addresses each clients needs and aims to reduce impairments. Refer individuals to appropriate follow-up care and services and assist with discharge and aftercare planning. Deliver psychotherapeutic interventions in line with professional and ethical standards. Collaborate with families, law enforcement, healthcare providers, and other community agencies around safety and care coordination. In the absence of the Clinical Manager, the incumbent will provide clinical and administrative support to team members. Offer consultation and informal supervision to other Mental Health Clinicians (licensed and unlicensed). Support documentation and crisis response training for staff as directed. Maintain service records, logs, and other required documentation; contribute to audit preparation and performance reporting. Assist with policy/procedure development and ongoing quality assurance activities. Represent the agency in community presentations and training related to mental health and substance use issues. Perform other duties as assigned to support the overall functioning and success of the program. May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible. What I bring Master's Degree: Social Work, Psychology, Counseling, or related behavioral health field. Certification: LPCC, LMFT, LCSW and BBS Registration Drivers License: Class C 2 years of professional experience working in mental health counseling, psychotherapy, and related social services in a public or certified private welfare or health agency. At least 1 year experience working with high risk and high-level acuity clients. Working knowledge of community and hospital resources. Working knowledge of Windows based computer applications to include Microsoft Word, Microsoft Excel, and Outlook. Working knowledge and familiar with EHR database systems containing PHI. Ability to quickly develop rapport with individuals seeking mental health services who are struggling with adverse life circumstances. Ability to support and verbally de-escalate clients during crisis encounters. Ability to assess individual's potential for dangerousness to self, others and/or grave disability. Ability to follow diagnostic criteria when determining pathology as outlined in the DSM-V, as well as be able to conduct MSE exams, CANS assessments, and identify determinants of health. Ability to appropriately and immediately document interactions with clients to Medi-Cal standards. Demonstrates emotional resilience, and the ability to remain calm and focused during high-pressure situations. Must be compassionate, ethical, and committed to providing client-centered care. A collaborative team mindset, adaptability, and a genuine dedication to supporting at-risk and high-risk population is essential.
