Apply today!Employment Type: Full-TimeSetting: Hospital-BasedShift: Night Shift (Tuesday-Friday)Compensation: The anticipated wage for this position is between $72 00 and $ 103.00Hourly wage depends upon experience, education, geographic location and other factors.We're seeking an RN Case Manager to coordinate the continuum of care and discharge planning for a designated patient population. This role focuses on ensuring high-quality, cost-effective care through collaboration with interdisciplinary teams, utilization review, and effective discharge planning.The Case Manager does not provide direct bedside care . Instead, they serve as a clinical and operational resource to ensure that patient services and treatments are delivered in the most clinically appropriate and financially efficient manner while meeting regulatory and payer requirements.Required Experience Minimum 2 years of nursing experience (required) Prior Nurse Coordinator experience in an OR or procedural environment (required) Required Knowledge, Skills, and Abilities Strong patient assessment, care coordination, and discharge planning skills Knowledge of utilization review, case management models, and quality improvement practices Understanding of hospital operations and California healthcare regulations Knowledge of reimbursement processes and public/private payer requirements Ability to analyze clinical and utilization data and maintain effective reporting systems Strong communication, negotiation, and conflict-resolution skills Ability to collaborate effectively with interdisciplinary teams and leadership Knowledge of medical terminology, levels of care, and the full continuum of care Understanding of factors impacting patient outcomes and healthcare delivery Licenses & Certifications Active CA Registered Nurse (RN) License California State Licensure and/or Compact State Licensure (required) Key Responsibilities Coordinate all aspects of patient admission, transitions, and discharge planning Manage patient movement across services and ensure appropriate handoff of care accountability Develop and implement timely discharge plans for patients with complex medical needs Collaborate with physicians, nurses, social workers, and interdisciplinary teams to assess and plan patient care Perform prospective, concurrent, and retrospective utilization reviews for appropriateness of admission, level of care, and length of stay Monitor length of stay and collaborate with providers to optimize resource utilization Communicate with third-party payers and review organizations to support reimbursement Provide education and consultation to patients, families, and hospital staff regarding discharge planning and case management Participate in quality improvement initiatives and clinical process improvement teams Maintain accurate documentation, utilization data, and compliance reports Participate in department planning, committees, and performance improvement activities May lead or support administrative and clinical staff assisting with case management functions Education Qualifications Bachelor's Degree from an accredited college or university (required) Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
