The Behavioral Health Manager provides clinical leadership of behavioral health and/or MAT clinical providers within Mosaic Community Health primary care community health centers while additionally working as part of an interdisciplinary team to provide behavioral health services to a diverse population. They will consult with primary care providers and other clinical team members on mental health, behavioral medicine, and substance use disorders. This role supervises the members of the behavioral health and/or MAT team, assignment will vary based on region and/or area of focus. Direct Patient CareProvides direct patient care at a minimum of 50% in a primary care clinic setting. Works with members of primary care team to assess, consult, treat, and manage patients with substance use, mental health, and behavioral health concerns within an integrated behavioral health care model. Performs intake, evaluation, consultation, treatment planning, brief interventions, and group treatment, documenting within Epic EHR system. Assists in the development of patient care plans to prevent further psychological or physical deterioration through referrals and moment-to-moment consultation. Directs individual and crisis intervention within behavioral health care model. Triages and refers patients to appropriate treatment modality (e.g. inpatient, out-patient, primary care-based treatment etc.) based on assessment. Consults with primary care team in management of patient care. Outreach to patients missing services based on guidelines / reports from patient registry or other sources. Deliver patient care within HRSA services scope and approved locations, encompassing patient homes, community-based sites, and providing home-based care when necessary and in adherence to defined services and safety protocols. Local travel required. Education/CounselingAssists patients and family, through individual, group, family, and/or couples brief interventions within the context of an integrated behavioral health model, teaching coping mechanisms, providing support and guidance. Conducts group classes and group visits to promote skill building, education, and improved health for the patient. Provides training and education to care team members to facilitate diagnosis and treatment of mental health, behavioral medicine, and substance use concerns within primary care. People ManagementManage performance of the team, conduct monthly meetups and provide real-time and consistent coaching for all direct reports. Ensure accountability of team, including but not limited to adherence with policies, procedures, guidelines, workflows, productivity and addressing attendance, performance, and behavior concerns including corrective action if necessary. Consult with Human Resources to support employee relations, performance achievement, training, employee engagement, etc. Attend team meetings to communicate organization, site, and/or department information, changes, initiatives, etc. Obtain leadership approval and manage all recruitment and hiring for team. Provide oversight and support for recruitment/hiring of openings supervised by direct reports. Effectively monitor onboarding of new hires, ensure on-going training and development for all staff. Assist with development and implementation of BH Internship program, as applicable. Manage time off requests and approve timecards for direct reports or as delegated and monitor to ensure staff takes required breaks and lunches. AdministrativePartners with Director of Behavioral Health, and other System Leadership Team members in developing substance abuse services within Mosaic’s primary care practices as appropriate for clinic setting. Champion the ongoing development and standardization of the MAT service line if applicable Maintains up to date knowledge regarding billing, coding and documentation practices. Partners with Director of Behavioral Health and other system Leadership Team members to develop workflows that optimize provision of integrated behavioral health services for clinical team members and patients. Attends and participates in Quality Improvement activities; identify opportunities for program improvement. If Providing support to MAT service line: oversite and development of components including but not limited to policies and procedures; KPIs; schedule/templates; community partnerships; training and education to best meet organization needs that align with evidence-based practice and FQHC guidelines Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices. Work Experience: Five years of experience delivering mental health and/or substance abuse services or related field work required. Experience with medically underserved populations required. People management and/or leadership experience of licensed provider preferred. Education, certification and licensure: Master's degree in a human services field (e.g. LCSW, LPC, LMFT) or doctorate in psychology (PhD/PsyD) and corresponding licensing-board registration required. Skills & Knowledge: Has the competencies necessary to: Identify precipitating events; Gather histories of mental, emotional and physical disabilities, alcohol and drug use, and past mental health services; Assess family, social and work relationships; Conduct a mental status assessment; Document a DSM diagnosis; Write and implement or supervise implementation of a treatment plan; Conduct and document a mental health assessment; Provide mental health treatment and rehabilitative services within ones scope of practice; Experience working with adult populations with addictions and complex issues. Execute evidence-based brief interventions. Ability to make quick and accurate assessments of mental health conditions related to substance abuse. Interest in gathering, tracking and presenting outcome data. Expertise in trauma-informed, culturally sensitive substance abuse and mental health treatment. Be knowledgeable of local and state resources. Be able to work with patients from infancy to geriatrics. Comfort with electronic medical records and charting. Flexible and able to assess and triage on the spot demands of medical staff.
