Role Responsibilities Deliver comprehensive, continuous care for patients throughout all life stages-from children to seniors. Effectively manage chronic illnesses, preventive care, acute medical issues, and mental health challenges with a patient-centered approach. Perform thorough physical examinations, analyze diagnostic results, and craft tailored treatment strategies. Collaborate with care coordinators, mental health specialists, and other healthcare professionals to ensure holistic patient support. Engage in initiatives aimed at improving quality of care and enhancing patient outcomes. Utilize electronic medical records (EMR) for meticulous patient documentation and communication. Provide care both in-person and through virtual platforms to adapt to patients' desires and situations. Education Requirements MD or DO from an accredited medical institution. Completion of an accredited residency in Family Medicine. Certifications Board Certification in Family Medicine. Eligibility for, or current possession of, a valid Illinois medical license. BLS and ACLS certifications are preferred. Skills and Experience Exceptional diagnostic and clinical abilities with a focus on prevention and proactive care.
