Travel Home Health Physical Therapist - $2,440 per week

Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 03/23/2026 Duration: 12 weeks 40 hours per week Shift: 8 hours Employment Type: Travel Title: Physical Therapist (PT) – Home Health Location: Port Charlotte, FL 33948 (Coverage area: Port Charlotte, Englewood, Venice, Nokomis, Osprey, Sarasota – up to 60-mile radius from hub) Duration: 13 weeks Shift: Day – 5x8-hour shifts (8:00 AM – 5:00 PM) Job Responsibilities: Provide in-home physical therapy services to geriatric patients. Manage an average of 5–a7 patients per day. Perform evaluations, develop care plans, and deliver skilled PT interventions. Complete OASIS documentation (required for clinicians performing SOCs). Document patient care in HCHB EMR (required). Travel throughout assigned territory (up to 60-mile radius). Maintain flexibility with territory coverage as needed. Participate in home health and/or hospice care as applicable. Ensure timely documentation and compliance with Medicare home health standards. Required Skills & Qualifications: Minimum 1 year of recent Home Health experience. OASIS experience required (for SOC clinicians). HCHB EMR experience required. Active state PT license in hand (license pending accepted for therapy only). BLS certification required. Ability to manage geriatric caseload independently. Proof of vehicle insurance required. Background check hits not accepted. Compliance items must be initiated within 24 hours of offer acceptance. RTO must be disclosed at time of submission. Home Health and/or Hospice competency checklist required. Traveler must be flexible with territory coverage. On-call may be required. First-time travelers accepted.
