Extended Hours Monday-Friday starting 7:30a-7p Extended Hours Saturday & Sunday starting 7:30a-7p Nurse Assessor Can Build Their Own Schedule Each Week Nurse Assessor Can Make Up Hours for Interrupted/No Shows Work Any Calendar Day Pay Rate: $55-$58/hr. Hourly wage may depend upon experience, education, geographic location and other factors. Qualifications New York State RN License, required. Work experience with UAS-NY or Oasis Community Assessment tool 2+ years' experience working in-home setting within the last 15 years, required. Work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities. Computer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; and proven ability to work with community groups. Knowledge of long-term care services and programs Ability to travel throughout the assigned region, as assigned, including travel to individuals' homes. Highlights Full county coverage required. Responsibilities The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians order. The incumbent may follow up with individuals who have received an assessment through face-to-face communications. Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports. Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes. Work collaboratively to monitor and address clinical quality and gaps in care. Provide clinical and educational assistance to support project initiatives. Demonstrate communication skills necessary to interpret age-specific responses.
