What You'll Do Provide skilled nursing care to patients in their homes Complete comprehensive assessments and develop individualized plans of care Monitor patient progress and adjust care as needed Educate patients and families on medications, treatments, and self-care Collaborate with an interdisciplinary care team (RNs, LPNs, therapists, caregivers) Maintain accurate and timely documentation using HomeCare HomeBase (HCHB) Participate in a rotating on-call schedule What We Offer Competitive pay: $35–$43/hour (DOE) Predictable weekday schedule with rotating on-call opportunities Mileage reimbursement AND paid drive time for all patient-related travel Comprehensive benefits, including: Medical, dental, and vision insurance Paid Time Off (PTO) Education stipend 401(k) retirement plan Ongoing training, clinical support, and team collaboration A non-profit, mission-driven environment!
