Key Responsibilities: Encourage effective communication between patients and the interdisciplinary team. Build and sustain therapeutic relationships through patient education and promoting self-advocacy. Utilize advanced technology to accurately document skilled services provided. Qualifications: Must be a Licensed Practical Nurse with a valid CT LPN license. At least one year of LPN experience; experience in homecare is preferred. A valid driver's license and reliable transportation are necessary. Proficiency in computer skills is required.
