Arise is looking for a Housing Specialist / Olmstead Housing Subsidy The Olmstead Housing Subsidy program provides support to individuals who wish to move from nursing home residence back to living in the community. The program provides support services and rental assistance to eligible individuals in Onondaga and surrounding counties. The Housing Specialist assists with transitioning out of institutional settings by locating and securing suitable housing and preparing the home for move in. The Housing Specialist works closely with the Independent Living Specialist to secure services which help maintain stable housing in a person's chosen community setting. This position serves primarily Onondaga, Oswego and Madison counties. Required Qualifications: • Associate's degree required, bachelor's degree preferred, or equivalent combination of education and related experience• Excellent communication and organizational skills• Experience working with individuals with disabilities required• Solid knowledge of available community-based services• Proficiency in Word and Excel, general computer literacy to navigate databases, use e-mail and other required electronic systems• Valid New York State Driver's License Essential Functions: • Work collaboratively with our Open Doors program staff to identify individuals in nursing homes that are eligible for the OHS. • Take direction from ARISE-EFR Housing Manager and the Technical Assistance Team at funder agency to meet all program requirements• Develop and maintain positive relationships with housing developers, apartment managers and landlords to further the understanding of the program and increase the availability of affordable and accessible housing. • Help identify strategies and goals for local outreach, and identify partnerships to be developed, both statewide and locally; provide community education to raise awareness of the program and its services• Accept and respond timely to referrals from nursing facilities, transition specialists and the New York Association on Independent Living (NYAIL.) • Determine OHS eligibility and if appropriate, search for housing that will meet the needs of the individual such as affordability, accessibility and location. • Follow OHS procedures for the inspection of apartments/homes and approval of subsidy. Work location: Olmstead Housing Subsidy -Madison County OfficeHours and days: Monday-Friday 8:00am -4:00pm Compensation: $20.16 -$21.16 per hour Non-Exempt - 40 hours per week, including one-hour paid lunch Checkr Background Check
