Duties include:Develop and update individualized client care plansConduct supervisory RN visits to assess condition and quality of servicesCollaborate with the Director to match care needs with the right servicesProvide hands-on caregiver training in our Prior Lake training centerEnsure caregivers demonstrate competency with delegated tasksReassess each client's needs and satisfaction every 90 days or as neededDocument any changes in client condition and adjust care plans accordinglyMaintain communication with clients and designated family members Why Join Griswold?
