Job Description

Join a dedicated healthcare team as a Licensed Practical Nurse providing compassionate patient care during night shifts in a medical-surgical setting. This role involves delivering direct care aligned with individualized patient plans and collaborating closely with Registered Nurses to promote patient comfort and recovery. Key Responsibilities: Provide patient care based on nursing processes within your scope of practice during assigned shifts. Support patients' physical, psychosocial, educational, and safety needs across various age groups. Gather and contribute accurate patient information to assessments, care plans, and discharge planning, involving patients and caregivers in the process. Communicate effectively with patients, families, healthcare team members, and physicians to ensure quality care and timely updates on clinical changes. Perform therapeutic procedures as assigned and document nursing interventions and patient responses thoroughly. Assist with patient admissions, discharges, and transfers while addressing psychosocial needs in collaboration with social services and discharge planners. Recognize and manage potential health risks by initiating appropriate actions in partnership with Registered Nurses. Collaborate with interdisciplinary teams to enhance patient care and seek guidance when necessary to maintain safe and effective nursing practices. Utilize communication strategies to achieve positive outcomes and patient satisfaction. Education: Completion of an accredited Licensed Practical Nurse program is required. Experience: Previous experience in a medical-surgical nursing environment is preferred but not mandatory. Requirements: Current LPN licensure, strong communication skills, ability to adapt to changing clinical situations, and commitment to patient-centered care.
