About the Job Mission PediatricsJoin Our Team as a LVN at Mission Pediatrics!Salary: $30-$35 per hour Location: 6950 Brockton Ave., Suite #4, Riverside, CA 92506 Position Status: In-Person, Full-Time Regular Estimated Weekly Hours: 30-40+ Bilingual (English/Spanish) preferred.Job Description: The LVN Nurse Care Manager provides comprehensive medical care management services within the Enhanced Care Management (ECM) Program. This role delivers care coordination, medication reconciliation, member education, transition planning, and interdisciplinary collaboration for pediatric members with complex medical conditions. The position promotes whole-person care principles and supports regulatory compliance and quality performance metrics. Essential Functions and Responsibilities:Manage a caseload of members with complex chronic medical conditions (primarily Tier 1 and Tier 2 as defined by ECM risk stratification). Conduct medication reconciliation and collaborate with pharmacy and primary care providers. Develop and implement Health Action Plans and Shared Care Plans. Coordinate transitions of care with hospitals and health plans. Monitor laboratory, treatment, and medication alerts. Track required assessments and outcome measures, including Comprehensive Health Assessments and Shared Care Plans, within care management platforms. Utilize motivational interviewing and patient activation techniques. Provide chronic disease education and lifestyle counseling. Participate in interdisciplinary case reviews, including Systematic Case Reviews and care conferences. Ensure regulatory compliance and timely, accurate documentation consistent with ECM program standards. Collaborate with community providers and ancillary services. Knowledge, Skills, Abilities, and Other Characteristics (KSAOs):Knowledge of pediatric chronic disease management and care coordination principles. Understanding of managed care environments and Medicaid/IEHP systems. Strong clinical judgment and communication skills. Ability to analyze clinical data and collaborate across healthcare teams. Culturally competent and family-centered care approach. Strong organizational skills with ability to manage multiple priorities and documentation timelines. Proficiency with electronic health records and web-based care management platforms. Ability to collaborate effectively within interdisciplinary healthcare teams. Education Requirements and Experience Requirements:Graduate of an accredited LVN program required. Active, unrestricted California LVN license required. Minimum three (3) years of pediatric care management experience required. Associate's degree in vocational nursing preferred. Bilingual (English/Spanish) preferred. Work Environment and Physical Requirements Work is performed in clinical, office, and community-based settings. Requires prolonged sitting, standing, and computer use.
