As a CenterWell Home Health nurse reporting to the Clinical Manager, you will deliver personalized, one-on-one care that empowers patients to regain their independence in the comfort of their homes. Collaborate with a dedicated group of physicians and clinicians to implement care plans that support patients' recovery and help them return to their cherished lives. In this Home Health LPN role, your responsibilities will include: Delivering skilled nursing care to patients in their homes, under the guidance of an RN. Collaborating with patients, their families, and the healthcare team to develop and implement individualized care plans. Educating patients and their families about disease management, treatment options, and self-care techniques to enhance their well-being. Maintaining accurate and timely records of patient care, including visit details, interventions, and outcomes. Establishing and maintaining communication with patients, physicians, clinical managers, and other healthcare team members regarding patient or family needs and status changes. Participating in care coordination and discharge planning. Make a meaningful impact with your skills! Required Qualifications: Current nursing license in the practicing state. Valid driver's license, auto insurance, and reliable transportation. Current CPR certification. Minimum two years of experience as an LPN/LVN in a clinical environment. Preferred Qualifications: Nursing experience in a Home Health or Hospice setting. Compensation: Pay ranges from $33.00 to $47.00 per visit/unit, with an annual base salary between $53,000 and $73,000. Additional Information: This position is classified as patient facing and requires participation in Humana's Tuberculosis (TB) screening program. Successful candidates will undergo screening for TB. This role also requires a valid state driver's license as part of Humana's driver safety program. Personnel must maintain liability insurance for their personal vehicle, meeting state minimum requirements or a minimum of $25,000 for bodily injury per person, $25,000 bodily injury per event, and $10,000 for property damage, or higher.
