In this home-based care role, you'll partner closely with patients, families, and interdisciplinary teams to promote safe, effective, and compassionate care for elders and adults with disabilities. You'll guide patients through clinical coaching, care planning, and resource coordination-helping improve health outcomes and enhance quality of life. Job specification Candidate requirements Job specification Conduct in-person home visits to assess patient safety, monitor changes in health, and provide clinical coaching-including medication reviews. Implement and monitor individualized care plans, offering guidance and education via our digital care app. Coordinate appropriate referrals to community and clinical resources. Establish strong, trust-based relationships with patients and caregivers to promote engagement and reduce caregiver fatigue. Document assessments, interventions, and patient outcomes in accordance with regulatory and agency requirements. Participate in team meetings and contribute to projects and initiatives as needed. Candidate requirements Active Registered Nurse (RN) License in the state of Massachusetts. Minimum 2 years' recent experience providing direct care to elders or adults with disabilities. Valid driver's license, reliable vehicle, and auto insurance. Experience with case management, care planning, and clinical assessments, ideally in eldercare or chronic disease management. Strong computer literacy, including Microsoft Office and case management systems. Bilingual proficiency is a plus and may lead to enhanced compensation. Why work with us?
