This full-time position as a Clinical Pharmacist in the NICU/Pediatrics department involves taking responsibility for the medication management of assigned patients. You will monitor and evaluate medication therapy, ensuring appropriateness based on patients' health statuses and institutional guidelines. You will engage in medication reconciliation, monitoring, and patient education. Additionally, you will play a key role in educating Pharmacy and Medical Residents, as well as healthcare providers, while contributing your pharmacy expertise to various documents and order sets. Education Requirements: Required: Bachelor of Science in Pharmacy and completion of a PGY1 Pharmacy Practice Residency (3 years of related specialty experience may substitute for PGY1 Residency). Preferred: Doctor of Pharmacy (PharmD) and completion of a PGY2 Specialty Pharmacy Residency. Work Experience: Required: None. Preferred: Prior experience with pediatric patients. Certifications: Required: Current Pharmacy license in the state of practice. Knowledge, Skills, and Abilities (KSAs): Proficient computer skills for data entry and retrieval, with the ability to work with Windows applications. Excellent verbal and written communication skills, capable of presenting information clearly to various audiences. Strong leadership and teaching skills. Key Responsibilities: Provide pharmacokinetic consultations, evaluate drug selections and dosing based on laboratory data, and communicate findings with healthcare practitioners. Evaluate patients' responses to drug therapies. Document interventions and conduct outcome studies on clinical activities. Assist in developing pathways, best practices, and guidelines for the patient population. Educate staff, students, patients, and other healthcare providers. Aim to submit for publication biannually. Mentor residents and new staff to achieve departmental objectives. Ensure continuity of patient care and support through all transitions of care. Adapt your approach to meet the special needs of diverse patient populations. Perform additional related duties as required. Physical and Environmental Demands: This role involves regular exposure to patient care environments and may include working directly with patients. The incumbent should be able to exert light to moderate physical effort, including moving objects weighing up to 20 pounds frequently and potentially up to 100 pounds occasionally.
