Your role will involve: Integrating evidence-based clinical guidelines and protocols into patient-centered transition plans Representing the organization in strategic partnerships with healthcare facilities and professionals Fostering relationships with hospitals, physicians, and executive-level stakeholders to enhance care delivery Qualifications Essential Qualifications: Graduate from an accredited nursing, therapy, or social work program Current licensure in your state of practice Two years of relevant field experience Knowledge of federal, state, and local regulations governing Medicare certified home health and hospice services Basic technology skills, including mobile device proficiency Preferred Qualifications: Registered nurse or physical therapist Three years of field experience Experience in home health or healthcare sales Requirements: Valid state driver's license Automobile liability insurance as mandated by law Reliable transportation in good working condition Able to drive safely in various weather conditions Additional Information Enhabit Home Health & Hospice is an equal opportunity employer.
