Role Responsibilities Deliver thorough, ongoing care for diverse patients spanning all age groups, from infants to seniors. Oversee the management of chronic illnesses, preventive care, acute health issues, and mental health conditions from a patient-centric perspective. Carry out comprehensive physical exams, analyze diagnostic tests, and create individualized treatment plans. Work collaboratively with care coordinators, behavioral health experts, and specialists to ensure comprehensive patient care. Engage in quality improvement projects aimed at enhancing patient outcomes and operational efficiency. Employ electronic medical records (EMR) to uphold precise and up-to-date patient documentation. Offer care through both in-person visits and virtual consultations to accommodate patient preferences. Education Requirements MD or DO degree from an accredited medical institution. Completion of a recognized Family Medicine residency program. Certifications Board Certification in Family Medicine. Eligibility for (or active) Illinois medical license. BLS and ACLS certifications preferred. Skills and Experience Strong clinical and diagnostic skills focused on preventive medicine.
