Job Title: Physical Therapist (Skilled Nursing & Rehab) Location: Multiple Locations - All States, USA Pay Range:-/hr (dependent on experience, location, shift) Employment Type: Contract or Full-Time Facility Type: Skilled Nursing Facility (SNF) or Rehabilitation Center Position Overview We are seeking dedicated Physical Therapists to join leading skilled nursing and rehabilitation facilities across the United States. The role entails providing high-quality, patient-centered therapy services to a diverse population encompassing both short-term rehab and long-term care residents. You will collaborate with interdisciplinary teams to develop, implement, and monitor individualized treatment plans that maximize physical function, mobility, and overall quality of life. Key Responsibilities Deliver comprehensive physical therapy assessments to both short-term and long-term care patients. Develop and implement individualized treatment plans addressing each patient's mobility, strength, pain management, and rehabilitation goals. Perform a variety of skilled therapy interventions including therapeutic exercise, gait training, balance and coordination activities, manual therapy, and modalities. Provide education to patients, families, and caregivers on home exercise programs, safety, and ongoing rehabilitation. Document all patient care in accordance with state/federal regulations and facility standards (including EMR/electronic charting as required). Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team. Participate in interdisciplinary team meetings, care conferences, and discharge planning processes. Monitor patient progress and adapt therapy plans to ensure optimal outcomes. Ensure compliance with all federal and state guidelines, including Medicare/Medicaid and SNF regulatory requirements. Foster a safe, respectful, and supportive environment for patients across all acuity levels. Required Qualifications Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy (DPT) or equivalent). Current, active state Physical Therapist license in good standing (or eligibility to obtain licensure in respective state). CPR/BLS certification. Previous experience in skilled nursing facility (SNF), inpatient rehab, or mixed short/long-term care settings preferred. New graduates welcome; prior SNF/rehab experience a plus. Strong communication, organizational, and interpersonal skills. Ability to work independently and as part of an interdisciplinary team. Knowledge of EMR/electronic charting systems desirable.
