Your responsibilities will include: Developing patient-centered transition plans based on evidence-based guidelines and protocols Promoting quality and efficiency in post-acute care delivery Building strategic relationships with health systems, hospitals, inpatient facilities, and healthcare professionals Qualifications Essential: Graduation from an approved nursing, therapy, or social work program A valid license to practice in your state At least two years of field experience Strong understanding of federal and state regulations governing Medicare-certified home health and hospice Basic technology skills, including mobile device operation Preferred: Registered Nurse or Physical Therapist certification Three years of field experience Previous experience in home health or healthcare sales Requirements Must possess a valid state driver's license Maintain automobile liability insurance as required by law Dependable transportation in good working condition Ability to safely drive in various weather conditions Additional Information Enhabit Home Health & Hospice is an equal opportunity employer committed to fostering a diverse, collaborative, and respectful work environment.
