Clean driving record (MVR) with no major violations: No more than 3 minor preventable accidents in the last 3 years. No more than 1 major preventable accident in the last 3 years. No DUI, DVI, or DWI charges in the last 10 years. Must comply with all FMCSA requirements. Must be free from any prohibitions in the FMCSA Clearinghouse.
